Enhanced Care Manegement- Coordination of services. A thorough intake is done to meet the members needs, a care plan is established and goals are set for the member and case manager to work on. Case Managers are advocates for members in the community. Housing Navigation- Case Managers work closely with the memebers to assist with housing, we cover application fees, secuirty deposits, first months rent, household items, and furniture. Our staff is dedicated to help serve their members with love, and dignity
